Configuration Instructions

These instructions are for the administrator configuring the program for transmitting images to the TRIAL principal investigator site.

Configure the Anonymizer

Click the "Anonymizer" tab.

The TRIAL parameter should be the name of your clinical trial.

The SITENAME parameter should be the name of your site.

The SITEID parameter should be configured from the following table:

SiteSITEID
Hospital ID-Hospital

The rest of the Anonymizer configuration is set correctly for the trial. If you need to make changes, see the Anonymizer Configuration Details section below.

Configure the Control Panel

Click the "Control Panel" tab. A second row of tabs will be displayed allowing you to configure the various functions of the program.

On the "Trial" tab, examine the Destination URL field. If your site can make a Secure Sockets Layer outbound connection on port 8443, the destination field is correct.

If it can't, remove the 's' in "https" and change "8443" to "8080".

If you aren't sure, there is no harm in trying the SSL connection first. If it doesn't work, you can return to the Control Panel and change it later.

Ensure that exporting is enabled.

On the "DICOM SCP" tab, examine the "DICOM SCP Port" and "DICOM SCP AE Title" fields and change them if necessary. In most situations, the values will be fine. If someother application on your computer is using the selected port, the DICOM SCP will fail to start and an error will be reported in the "Event Log" tab. In that situation, you will have to change the port number.

On the Control Panel's "Anonymizer" tab, ensure that the anonymizer is enabled.

If your trial is using central remapping, check the "Central Remapper enabled" box and enter the URL of the Central Remapper in the field below it. If you are using HTTP to communicate with the Central Remapper, the URL will typically have the form http://ipaddress:8080/storageservicename/remap. If you are using HTTPS, the URL will have the form https://ipaddress:8443/storageservicename/remap.

"Anonymizer force IVRLE" should be unchecked unless directed by the trial administrator at the principal investigator site.

When you are done making changes, be sure to click the "Save Changes" button. Changes do not become active until they are saved.

Test Image Transmission

Click the "Parameters" tab and note the Local DICOM Store parameters.

Using those parameters, configure a DICOM modality or PACS workstation and send some images.

Click the "Event Log" tab and see what happened.

Click the "Queue Status" tab and verify that all the queues and the quarantine are empty.

If images are in the Awaiting Export queue, check the log and see if it is because a connection could not be made. If it couldn't, two possibilities should be tested. The simplest test is to see if you can connect using HTTP rather than HTTPS as described in the section above. If that also fails, it is possible that your computer is behind a proxy server that does not allow direct outbound connections. If that might be the case, read the section below on configuring FieldCenter to use a proxy server.

If images are quarantined, read the section below on using the quarantine.

The Encryption Key

The re-identification system which is part of the anonymizer encrypts the keys in its database to make it more difficult for a hacker to acquire protected health information. The encryption key is a string of text that can be thought of as a password. You have the option to define your own key or to use a default key that is stored in the program. A good key is about 20 characters long, not counting any spaces.

When the program starts, it looks for an existing re-identification database. If it finds one, it checks to see whether the key to the database is the default. If it is not, it prompts the user for the key.

If your system is on a secure network, then using the default key may be convenient because the prompt is suppressed. If your system is in a less secure place, you should define your own key the first time the program is run. Once the key for a database is defined, it cannot be changed. To start a new re-identification database, you must remove the idtable.properties file from the FieldCenter directory and restart the program, causing it to create a new database and request a key the next time it runs.

Using the Quarantine

When a failure occurs while anonymizing or transmitting a DICOM object, the object is placed in the quarantine. No further processing of such an object will occur without manual intervention.

If objects have accumulated in the quarantine, click the "Quarantine" tab, click the "Open" button, and open an object. The image and all its DICOM elements will be displayed.

You can send the currently opened image from the quarantine by clicking the "Send" button.

You can send all the quarantined images from the same study as the currently opened image by clicking the "Send All" button.

You can also use the "Quarantine" tab to edit the contents of certain DICOM elements in the object. In trials where the anonymizer checks certain elements to determine their validity, it can quarantine an object that has missing or incorrectly formed values in those elements. If you click on the DICOM element number in the right pane of the Quarantine display, you can change the value of the element.

If you have made changes to any elements in an image and you wish to save them in the image, click the "Save" button. If you wish to propagate those changes through all the images in that patient's study, click the "Save All" button.

A typical use for this function is in correcting a missing or improperly formatted patient ID. For example, suppose the TRIAL patient ID starts with at least six numeric digits and the patient ID is placed in the Study Comments element by the modality operator. If the anonymizer finds that the element is missing or the first six characters are not numeric, it quarantines the image. By opening the image in the "Quarantine" tab, you can review the DICOM elements and edit them. The display highlights in red all the elements that could have resulted in the quarantine. After placing the correct ID in the Study Comments field, you can click the "Save All" button and correct all the images in the study. Then you can send them all by clicking the "Send All" button.

Using the Selector

The FieldCenter application is designed for unattended operation under normal circumstances, where other applications transmit objects to FieldCenter for processing and export. If manual selection of local objects is necessary, the Selector tab can be used to navigate local directories and select objects for processing. The Selector has a Directory pane on the left for selecting individual objects or directories to search, and a right pane for choosing object types to accept and for reporting results.

The checkboxes in the lower left side of the right pane allow selection of the object types that will be accepted for queuing.

The Queue button triggers a search for acceptable object types. Which files are selected depends on whether a file or a directory is selected in the tree and on whether the Include Subdirectories checkbox is selected in the lower left of the Directory pane:

Queuing events that occur during the search are logged in the upper part of the right pane.

Notes: In order to handle large objects like zip files of more than 100MB, the program requires more than the default memory allocation. To provide that memory, start the program from a command line as follows:

For object sizes greater than 400MB, allocate even more memory, as follows: The mx parameter specifies the maximum size in MB of the memory pool (also called the heap space), and the ms parameter specifies the starting size of the memory pool.

Configuring FieldCenter to Use a Proxy Server

The FieldCenter program is preconfigured not to go through a proxy server when establishing a connection to the principal investigator site. If your site is connected to the internet through a proxy server, take these steps:
  1. Click the "Control Panel" tab.
  2. Click the "Proxy" tab.
  3. Check the box enabling the proxy server.
  4. Enter the IP address or domain name of your proxy server in the "Proxy Server IP Address" field.
  5. Enter the port number of your proxy server in the "Proxy Server Port" field.
  6. If your proxy server requires authentication, enter the username and password in their respective fields. (Most proxy servers to not require authentication.)
  7. Click the Save button at the bottom of the page and test image transmission again.
When FieldCenter fails to make a connection, it assumes that the network or destination has a problem and waits 10 minutes before trying again. If you don't want to wait, you can exit the program and start it again.

Configuring FieldCenter to Use an Update Server

An update server is a central repository for configuration files and software releases, providing automatic backups of critical information, including the anonymizer scripts and the encrypted remapping database. It also provides a convenient method for updating the FieldCenter software.

If the principal investigator site in your clinical trial provides an update server, take these steps:

  1. Click the Control Panel tab.
  2. Check the box enabling the update server.
  3. Enter enter the URL of the update server in the field below it. If you are using HTTP to communicate with the update server, the URL will typically have the form http://ipaddress:8080/storageservicename/update. If you are using HTTPS, the URL will have the form https://ipaddress:8443/storageservicename/update.
  4. Obtain the username and password for your site from the principal investigator's trial administrator and enter them in their respective fields.
  5. Click the Save button at the bottom of the page.

Anonymizer Configuration Details

The anonymizer is highly programmable. If you want to change its actions to meet special IRB rules for your site, read "How to Configure the Anonymizer for MIRC Clinical Trial Services", available on the RSNA MIRC site at http://mirc.rsna.org. Click the "Downloads" button in the page header to go directly to a page with all the MIRC documentation.

The Anonymizer tab displays the anonymizer scripts. You can edit the scripts and click the Save button to save your changes. Changes are not effective unless the scripts are saved. If you make a change and decide you made an error before saving, click the Reset button and the scripts will be refreshed from the last saved version.

If you change the anonymizer scripts, do not change the contents of the ReferringPhysicianName field. The anonymizer uses this element, which is not used in the trial, to indicate that it has already processed a file.

The keyword this may be used in place of an ElementName to indicate the element whose replacement is being constructed.

The XML Anonymizer

The XML anonymizer is driven by a script file named "xml-anonymizer.script" located in the same directory where the FieldCenter program is stored. This file is not delivered as part of the installation, so you must construct it yourself using any text editor (e.g., TextPad). Because XML is very general, the script language is different from that of the DICOM anonymizer. It is also different in order to make it look XPath-like for the benefit of XSL wizards.

The following is a quick reference for the scripting language. There are three types of commands. Each starts on the first character of a line. Each is indicated by a specific starting character. A line not starting with one of the three command-start characters is appended to the preceeding line.

If no bracketed qualifier is present in a path segment, the first element matching the segment name is selected. If all elements matching the segment name are to be selected, use the "[*]" wildcard qualifier, e.g. /root/element[*].

To assist in debugging XML anonymizer scripts, there is a special name assignment command:
